This privacy statement discloses the privacy practices for www.homecarematters.org. If you have questions or concerns regarding this statement, you should contact one of the senior managers at the end of this disclosure.

The HomeCare Matters Home Health and Hospice Website is intended to serve as a means to inform users about hospice care for terminally ill persons and to educate individuals on other end-of-life and home health care issues. In the course of providing this online service, information is exchanged back and forth between HomeCare Matters and online users. In some places on this site you may be asked to list your name, address and other information. HomeCare Matters is sensitive to the private nature of information you provide to us over the Internet. Our Privacy Policy is designed to maximize your ability to protect your personal information while at the same time giving you the opportunity to obtain useful resources in end-of-life issues at reasonable prices.

Collecting and Disclosing Personal Information
HomeCare Matters Home Health and Hospice must collect certain personal information, such as your name, address, e-mail address, organization, telephone number, etc., in order to provide the products and services offered through the homecarematters.org Web site (the “site”).
HomeCare Matters is the sole owner of the information collected at the site. We will not share this information with any third party. This restriction is to include selling, reselling, trading, leasing or renting any personally identifiable information about you or your organization.
Users who sign up for our e-mail lists will receive regular communications that are explained on the sign-up page. We will in no case share e-mail addresses with other organizations.

Donations and Return Policy
If you donate to HomeCare Matters online, your name will be entered onto our database and you may receive occasional fund raising appeals via U.S. Mail. Your e-mail address will be treated as described in the paragraph above. It is the policy of HomeCare Matters to not sell, trade, lease or rent the names of its donors. Neither does HomeCare Matters buy the names of donors of other organizations. If you do not want to receive mailings, you can request by e-mail or in writing that we remove your name from our donor base.

If you believe that an error has been made in connection with your donation or memorial brick purchase, contact us immediately at 419-468-7985, 1-800-4-Nurses or hcmaccounts@rrohio.com . We will be happy to assist you in correcting any such error within reason, and if necessary make any refund that is appropriate.

Some links on the Site will take you directly from the Site to another Web site. The HomeCare Matters privacy policy is not applicable to any other Web site, and other Web sites may have their own policies on privacy that may differ from the HomeCare Matters policy.
